Improve Communication, Interactions by Using HPP's Quality Improvement Checklist

Health Partners Plans (HPP) is committed to helping our members maintain and improve health outcomes.

The Health Outcomes Survey (HOS) is an annual survey that asks Medicare Advantage members to detail their interactions with their physicians and other providers. The survey also asks Medicare beneficiaries if their providers addressed their health concerns. The results of the survey help evaluate how Medicare members view their current health status and determine if providers addressed their health concerns.

HPP encourages providers to use the quality improvement checklist, which can be found on the following page, to improve communication with patients and enhance patient-provider interactions. Click here to download the checklist now.

Please use this checklist as a guide for pre-visit prep or during visits with your patients. It includes tips for improving quality health outcomes during office visits.
