Health Partners Plans and ACLAMO Partnership Improves Access to Care in The Greater Philadelphia Area

More than 650 members have received support through the collaboration, with plans to expand in 2023

Philadelphia, PA — Health Partners Plans (HPP), an award-winning, not-for-profit health maintenance organization serving Pennsylvania and New Jersey residents, recently partnered with ACLAMO, a community non-profit that provides educational programs, social services and access to health and wellness programs to reduce health disparities for Latino, Black, and other minority communities.

Through this partnership, ACLAMO provides Health Partners Medicaid members with essential resources and medical services, while promoting healthy behaviors. Health Partners Medicaid members can contact ACLAMO to set up doctor’s visits and dental care, schedule preventative health screenings, access HPP’s care management service, schedule transportation to medical appointments and connect to community resources including food, clothing and utility payment services.

“Health Partners Plans is deeply committed to the local communities we serve, and we realize that many underserved populations experience gaps in their quality of health, and health care,” said HPP president and CEO Denise Napier. “Partnerships with well-respected community organizations like ACLAMO help to close these care gaps by connecting members who may be facing these disparities with needed health care and resources to ultimately improve their quality of life.”

To date, the partnership has impacted more than 650 HPP members, helping them to schedule annual physicals, seek mental health care and find support for rental and utility assistance. In 2023, HPP and ACLAMO will expand their efforts to reach and connect with new members and strengthen current relationships among their shared membership. There will be a focus on disease specific education, referrals to SDoH resources and reduction of health disparities by connecting members to care.

Any HPP member enrolled in a Health Partners Medicaid health plan can opt in to receive support from ACLAMO at no cost.
