Health Partners Plans is one of a few hospital-owned health maintenance organizations in the nation providing free and low-cost high-quality health insurance through our Medicaid, Medicare and CHIP plans. We help residents of Southeastern and Central Pennsylvania lead healthier lives through innovations and services that improve access to high-quality care.
Founded more than 35 years ago, Health Partners Plans continually develops new ways to encourage better health outcomes and has received national recognition for our innovations in managed care.
In November 2021, Jefferson Health became the sole owner of Health Partners Plans.

Our Health Plans
- Health Partners is our HealthChoices Medical Assistance plan serving Medicaid beneficiaries. Health Partners received a rating of 4.5 out of 5 from the NCQA Medicaid Health Plan Ratings 2021. This makes Health Partners one of the highest rated plans in Pennsylvania! Health Partners provides comprehensive benefits to adults and children on Medical Assistance, including doctors visits, immunizations, dental coverage and more.
- Health Partners Medicare offers three fast-growing Medicare Advantage plans to residents of Berks, Bucks, Carbon, Chester, Dauphin, Delaware, Lancaster, Lebanon, Lehigh, Montgomery, Northampton, Perry, and Philadelphia. Our Medicare Advantage plans provide more benefits than Original Medicare with no or low monthly plan premiums. CMS recently awarded Health Partners Medicare 4 stars out of 5 in their annual Star Ratings program.
- KidzPartners is our Pennsylvania Children’s Health Insurance Program (CHIP) plan for children up to the age of 19, who are uninsured and not eligible for Medicaid. In 2021, KidzPartners expanded and is now offered in Berks, Bucks, Chester, Dauphin, Delaware, Lehigh, Montgomery, Northampton, Philadelphia and Schuylkill counties.
Participating hospitals: Network of over 60 hospitals that have contracted with us to provide services to our members.
Participating providers: More than 6,400 primary care and specialist physicians and allied health professionals.
