Specialist Referral Requirement – HP Connect Provider Portal

Please be aware that effective July 1, 2018, all specialist referrals for our Health Partners Medicare members must be submitted through HP Connect, our provider portal. We will no longer process referrals submitted via fax or by phone.

If you have questions regarding referrals, need assistance setting up or accessing your account or have other needs, please email MedicareReferrals@hpplans.com. A representative will gladly assist you.

Referral Background

As communicated previously, effective January 1, 2018, HPP began requiring specialist referrals for most Health Partners Medicare members.*

* This requirement includes all Health Partners Medicare Special (HMO SNP) members as well as Health Partners Medicare Prime, Health Partners Medicare Value and Health Partners Medicare Basic members who reside in Philadelphia, Bucks, Chester, Delaware and Montgomery counties.

Specialist referrals are not required for those members enrolled in our Health Partners Medicare Prime and Health Partners Medicare Value plans who live in Lancaster, Lehigh and Northampton counties.
