About Our Plans

Health Partners Plans Medicaid

Provides health coverage for individuals with limited financial resources or meet other eligibility criteria as required by the Pennsylvania Department of Human Services.

Health Partners Plans CHIP

Insurance coverage for Pennsylvania children and teens under 19 years old who are uninsured and not eligible for Medicaid. 

Jefferson Health Plans Medicare Advantage

Provides the full benefits of original Medicare plus additional benefits including dental, vision, prescription drug coverage, and much more.

Jefferson Health Plans Individual and Family Plans

Coverage provided through the Affordable Care Act (ACA) for those that do not have coverage through an employer, Medicaid, Medicare, or CHIP.
