What is changing?
Health Partners Plans Medicaid will soon be called Jefferson Health Plans EverWell.
When will this occur?
We expect the name change to take place in the summer of 2026.
Will I still have the same benefits?
Yes. Your coverage, benefits and services stay the same. We will continue to offer programs that support your wellbeing and our teams will continue to help you navigate your health care.
Other than the name, nothing is changing with your plan.
Will I need to pick a new doctor?
No. You can keep seeing the same doctors and providers, as well as others that are part of our large and growing network.
Will I receive a new ID card?
Yes. Once the name change takes effect, you will receive a new card in the mail. Until then, keep using your current ID card. Both ID cards are valid through 2026.
Why is the name changing?
Our Medicaid and CHIP plans will join our Medicare Advantage and Individual and Family plans under the Jefferson Health Plans name.
Is the "Health Partners Plans" name going away?
Yes. We will soon stop using the Health Partners Plans name. But there may be a period of time when you see both the Health Partners Plans and the Jefferson Health Plans names. They refer to the same plan.
