Health Partners Plans Medicaid will soon be called Jefferson Health Plans EverWell, and Health Partners Plans CHIP will soon be called Jefferson Health Plans CHIP.
Does this change my status as a provider in the Health Partners Plans Medicaid and/or CHIP networks?
No. There is no impact to your status in our Medicaid and CHIP networks.
When is it happening?
We expect the name change to be implemented in the summer of 2026.
Are my contracts or reimbursement affected?
No. Existing contracts, reimbursement, credentialing, and payment processes remain unchanged.
Will contact information change?
No. All phone numbers and points of contact will remain the same. Our teams will continue to provide you with timely support and problem-solving.
Will members’ benefits or networks change?
No. Members will keep the same benefits, providers, hospitals and care teams.
Will members receive new ID cards?
Once the name change takes effect, members will receive a new Jefferson Health Plans ID card in the mail. Until then, expect members to keep using their current Health Partners Plans ID card. Both ID cards will be valid through 2026.
Why the name change?
Our Medicaid and CHIP plans will join our Medicare Advantage and Individual and Family plans under the Jefferson Health Plans name.
