Antidepressant Medication Management

Clinical depression is a common condition that may significantly affect your patients’ lives. Depression can cause many complex challenges and barriers to overcome. Antidepressant medication is often a key component of the therapeutic treatment plan and medication adherence remains a significant challenge.

We offer resources to assist you in managing our members with a diagnosis of major depression including our Antidepressant Medication Management Resource Guide and Depression Management Patient Tip Sheet.

Antipsychotic Medication Adherence Tools

In collaboration with Community Behavioral Health (CBH), we developed adherence materials to provide to members who are prescribed antipsychotic medications, including our Antipsychotic Medication Guide and Member Medication Guide.

Medication Adherence for Medicare Advantage Members

Jefferson Health Plans Medicare Advantage members may receive a call from these contracted programs on behalf of our team.

To learn more, download our Provider Resource Guide: Medication Adherence for Medicare Members, which includes information about Magellan Rx, Tabula Rasa, medication management support and other home delivery services.

Medication Reconciliation Post-Discharge

Medication reconciliation—the process of comparing a patient’s medication orders to all the medications the patient has been taking—is critical to the health of your patients and can help prevent medication errors such as omissions, duplications, dosing errors, and drug interactions. To learn more, download our Medication Reconciliation Post-Discharge tip sheet.
