As a Health Partners Plans member, you can manage your care anytime, anywhere with our secure online Member Portal.

In the portal, you can:

  • Find Care – Search for doctors, specialists, and hospitals, or update your PCP.
  • Access ID Cards – Request a new card or print a temporary one.
  • Check Benefits & Claims – Stay informed about your coverage.
  • Manage Your Health – Learn about common health conditions and how we can help.
  • Send Secure Messages – Contact us with questions about your plan

In-Person: First Friday of every month at 1 p.m. at our Community Wellness Center located at 6232 Market Street in Philadelphia.

Virtual: Fourth Friday of every month at 1 p.m. via our online link

Where to Get Care

As a Health Partners Plans member, you have many care options. It is important to know when and where to get care when you need it. Learn more

Virtual Care – 24/7 access through JeffConnect or Teladoc

Use for: Common medical concerns like colds, coughs, fevers, and digestive issues

Primary Care Provider – Routine care

Use for: Yearly well visits, vaccinations, sick visits, and ongoing care. Your PCP can also refer you to a specialist, as needed, for treating a specific medical condition.  

Urgent Care – Walk-in appointments and extended hours

Use for: Minor allergic reactions, asthma attacks, sprains, cuts. 

Emergency Room – Life-threatening emergencies

Use for: Signs of heart attack or stroke, major injury, and other medical emergencies

Get and Stay Healthy

Enhanced Member Supports Unit (EMSU)

Our EMSU offers assistance with health care and community services you need to stay healthy and/or manage a health care condition(s). Participation is voluntary. You may opt out at any time.

Here are examples of services available through our EMSU Care Coordination team

  • Support to help you understand the benefits you receive as a Health Partners Plans member.
  • Assistance in setting up doctor and dental appointments.
  • Access to food resources for you and your family.
  • Help with transportation needs.
  • Connection to behavioral health supports.
  • Housing support/utilities assistance.
  • Help to quit smoking or using other tobacco products.
  • Education on chronic conditions and the importance of preventive testing.
Health Survey

As a new member, we encourage you to complete a Health Survey. Your responses help us understand your healthcare needs and can link you to resources and programs to support you.
