Welcome to Health Partners Plans! Thank you for choosing us to provide your child's healthcare coverage. We're excited to have you as a member and look forward to supporting your family's health and well-being.

As a Health Partners Plans member, you can manage your child's care anytime, anywhere with our secure online Member Portal.

In the portal, you can:

  • Find Care – Search for doctors, specialists and hospitals, or update your child's PCP.
  • Access ID Cards – Request a new card or print a temporary one.
  • Check Benefits & Claims – Stay informed about your child's coverage.
  • Manage Your Child's Health – Learn about common health conditions and how we can help.
  • Send Secure Messages – Contact us with questions about your child's plan.

Do you know where to get care?

It is important to know when and where to get care when it's needed.

As a Health Partners Plans member, your child has access to many care options which include visiting a primary care physician, scheduling a telehealth visit, going to an urgent care center or visiting the closest emergency room.

Helping Your Child Get and Stay Healthy

Our care management team offers assistance with health care and community services your child may need to stay healthy and/or manage a health care condition. Participation is voluntary and you may opt out at any time.

Here are examples of services available through our Care Coordination team:

  • Support to help you understand the benefits your child can receive as a Health Partners Plans member
  • Assistance in setting up doctor and dental appointments
  • Access to food resources for your family
  • Help with transportation needs
  • Connection to behavioral health supports
  • Housing support/utilities assistance
  • Education on chronic conditions and the importance of preventive testing

If you would like to connect with the care management team, please call 1-866-500-4571 and enter your child's member ID when prompted. TTY users can call 1-877-454-8477.
