Coming Soon - Health Partners Plans Name Change

Health Partners Plans CHIP will soon be called Jefferson Health Plans CHIP.

We expect the name change to take place in the summer of 2026.

Yes. Your child’s coverage, benefits, and services stay the same. We will continue to offer programs that support their wellbeing and our teams will continue to help you navigate their health care.

Other than the name, nothing is changing with your child’s plan.

No. Your child can keep seeing the same doctors and providers, as well as others that are part of our large and growing network. 

Yes. Once the name change takes effect, a new ID card will be sent in the mail. Until then, keep using your child’s current ID card for their care. Both ID cards are valid through 2026.

Our Medicaid and CHIP plans will join our Medicare Advantage and Individual and Family plans under the Jefferson Health Plans name.

Yes. We will soon stop using the Health Partners Plans name. But there may be a period when you see both the Health Partners Plans and the Jefferson Health Plans names. They refer to the same plan.
