HPP University

Health Partners University is a series of online educational offerings targeted to the needs of various Health Partners Plans audiences.

We invite you explore these interactive presentations designed specifically for our providers. We also ask that you encourage your colleagues and practice staff to take advantage of these online learning opportunities.

EPSDT for Providers

Early and Periodic Screening and Diagnostic Treatment (EPSDT) is a valuable component of patient care for children under 21. Model of Care Training for HPP Providers

The Centers for Medicare & Medicaid Services (CMS) requires that all participating Medicare Advantage Organizations providers involved in the care of our dual-eligible special needs plan (D-SNP) members complete a D-SNP MOC training. At least one member of a care team location is required to take the annual online training course about the special needs model of care program. The individual(s) completing the course are responsible for distributing the training material to all D-SNP care team members. Our goal is to ensure that care team members understand how the program works, and the role that each member plays in the care of our members. Successful completion of this training module is mandatory.
