Controlling Blood Pressure

Health Partners Plans (HPP) would like to inform you about resources and information related to blood pressure management. Hypertension is one of the most common chronic conditions among the HPP population. HPP has identified opportunity for improvement in controlling hypertension, which may also lead to better performance and increased QCP incentives for providers.

HPP recently developed a Controlling Blood Pressure (CBP) Education and Resource Guide that is intended to help you and your care team:

  • Learn more about the CBP measure
  • Understand how you can impact your QCP incentive
  • Discover how HPP can support you and your care teams

Additionally, HPP has identified a list of your patients that are currently non-compliant for the CBP measure. The list is prioritized by those who have hypertension, had a visit with you in 2022 but are non-compliant for the CBP measure because HPP does not have a blood pressure reading in calendar year 2022 for these patients. Please review the list and consider submitting a script for an electronic blood pressure cuff for these patients. Both Medicaid and Medicare HPP Members have benefits for a blood pressure cuff. The blood pressure cuff order form can be accessed at HPPlans.com/cuff.

Your HPP Provider Relations Representative will send the CBP Education and Resource Guide and outreach list to you. As a reminder, patient-reported blood pressure readings during telehealth visits or telephone assessments are permissible.
