Medicare Diabetes Prevention Program (MDPP) Sites Expanded

Health Partners Plans is excited to announce that we've contracted with the YMCA of Delaware in Wilmington to offer our new Medicare Diabetes Prevention Program (MDPP) benefit. We wanted to be sure that you knew this site is now available to your patients in communities near Wilmington.

We also want to remind you that Temple Health, our original MDPP provider, has classes at multiple locations throughout the community, including one offering a bilingual program for Spanish-speaking members.

As you know, this new CMS-approved benefit is designed to help prediabetic Medicare members make appropriate lifestyle changes to help reduce the risk of developing type 2 diabetes. Our members who qualify for this one-time benefit will have no cost sharing throughout the duration of the program, which can be up to two years.

Our MDPP benefit can help your patients through:

  • Coaching by a certified specialist to learn how to eat healthier, boost physical activity and lose weight
  • Help with problem solving, managing stress and lifestyle changes
  • Weight control classes and other support services

We have also written to selected members who may be at risk for developing diabetes to tell them about this program, and advised that seeing their PCP for evaluation is the first step toward participation. Please keep this benefit in mind as a valuable resource for your prediabetic Health Partners Medicare patients, especially those in the Philadelphia and Wilmington areas who would find these sites to be convenient.
