CCMP Updates Scheduled for September 16, 2019

Please be aware that effective September 16, 2019, an improved Chronic Care Management Program (CCMP) tool is being reintroduced* on our NaviNet provider portal.

The updated CCMP tool features a number of improvements that enhance your experience, including changes that ensure an efficient and compliant process to appropriately capture, report and validate accurate diagnosis coding.

In addition to the return of diagnosis code validations, CCMP features design improvements, new functionality and more robust data processing, which enhance the value of this tool for your office.

To highlight these improvements, HPP will be conducting a CCMP webinar on Wednesday, September 25, 12:30 p.m.–1:30 p.m. To register for this webinar, visit and click “Register.” You will receive an email that confirms your registration and gives you the option to add the webinar to your electronic calendar.

In addition to the webinar, your Network Account Manager (NAM) will be scheduling on-site visits during the next couple of months to review these changes with your staff.
