Please take our HPP Provider Communication Survey
Please take a few minutes of your time to complete our online survey intended to solicit your thoughts and ideas about how Health Partners Plans (HPP) can best communicate with your office.
As an incentive for completing the survey, each completed response will be enrolled in a drawing for a new Apple Watch!*
Please note that we are interested in receiving feedback from any individual who has a recurring role in receiving, sharing and/or acting upon information communicated by HPP, and we welcome more than one response per office. Average time to complete the survey is less than 10 minutes.
HPP and other organizations in the health care field send communications to address a significant number of topics, many of which are driven by regulatory and contractual requirements. In addition, HPP aims to inform providers regarding new and updated policies, programs and procedures that impact providers and our members/your patients.
At the same time, we know the volume of communications can be a challenge, so we want to make sure we’re communicating effectively with you. If we aren’t communicating in a way that best fits your needs, tell us. We want to fix it. Your feedback is important and will be used to improve the way we connect with you.

* The recipient of the Apple Watch will be selected in a random drawing from all eligible entries received by Friday, November 16. Please note that your response can be anonymous; however, in order to be eligible to receive the Apple Watch you will need to complete your contact information (name, phone/email address, group name and Tax ID #).
